Even though I know what "out of pocket" means (away), I have no idea why it means it?

I'll go with the notion that the one saying it is going to take their cellphone out of their pocket and become unreachable by voice, text, or mail.

Actually it means an expense you have to pay up front as opposed to having someone else cover it. You literally need to take it out of your pocket.
